Homemaker Nurse Supervisor
The Nurse Supervisor position requires the individual to be licensed as a Registered Nurse or Licensed Practical Nurse with two or more years' experience in a home health care setting. They will come with training, education, and experience in Home Health Care tasks/services in order to supervise the provision of such tasks in accordance with accepted standards of care. He/she is responsible to the Director and/or immediate supervisor.
The Nurse Supervisor is responsible for all required nurse visits to the client's home and overseeing the care rendered. He/she is primarily responsible for initiating and the development of the Care Plan. The Nurse will perform the initial face to face to assess the client and his environment for appropriateness. The services should begin within 48 hours of the initial assessment. Upon initial evaluation, the Nurse Supervisor should initiate the agency's Service Plan within 48 hours. She should assure all care delivered is done.
The Nurse Supervisor will assure the worker is able to perform all tasks assigned. The Nurse Supervisor is responsible for assuring services are supervised in the clients home within thirty days after his/her initiation of services, then, no less than every 180 days after the initial start date.
During the follow-up visit Nurse Supervisor should re-evaluate for the following:
1. Client overall condition
2. Quality of care being rendered
3. New problems, Problem approaches, and responses
4. Updates needed for the SW
The Nurse Supervisor will communicate any new updates or changes to the appropriate agency staff.
