Licensed Social Worker - St Leonard - Exceptional New Hourly Rates
Working with the interdisciplinary team, the Social Worker assesses and promotes the social and emotional health and well-being of residents and their families. The Social Worker provides service to adults and older adults with the goal of affirming the resident’s integrity, growth, and control over the qualities of their life.

Essential Job Specific Duties/Responsibilities
1. Demonstrates knowledge of skills necessary to identify specific resident needs and provide appropriate services.
a. Completes the social service portion of the MDS within fourteen days of admission.
b. Documents the social care component of the comprehensive care plan for each resident as identified by the interdisciplinary team within seven days of completion of MDS.
c. Notifies resident families of Medicare/Medicaid regulations, answering question and gathering information for them as needed.
2. Participates in development of interdisciplinary plan of care.
a. Meets with resident and/or family to gain information and insight to write social history and discharge plans.
b. Participates in care conference as appropriate along with others on interdisciplinary team.
c. Notifies resident/family of Medicare/Medicaid stipulations for discharge or continued stay.
3. Actively develops one-on-one relationships with residents and families as needed to be able to meet individual needs.
a. Makes determinations regarding strengths, weaknesses, and needs based on psychosocial assessments.
b. Counsels residents and families in dealing with feelings about death and dying and other emotional, mental, environmental, or physical limitations.
c. Participates in resident/family activities designed to promote social interaction, reality orientation, and intellectual stimulation.
d. Documents all resident/family interactions that would help other disciplines gain increased understanding of resident concerns/needs as appropriate.
e. Is responsible for acting as a liaison with social, health, and community agencies for appropriate referrals.
f. Acts as an advocate for each resident and their rights, empowering them to be as independent as possible.
4. Provides education and resources, as well as support to residents, families, and staff.
a. Acts as an information referral base for Medicare/Medicaid and insurance questions.
b. Interacts effectively with disciplinary team.
c. Actively takes a leadership role in teaching resident rights, abuse, and confidentiality to new employees at facility orientation and education days.
d. Complies with all educational in-service requirements mandated by the facility, external accrediting, and regulatory agencies.
5. Participates and takes a leadership role as needed in various activities.
a. Oversees complaints/grievances by keeping current activity log of such and assisting with final solution/outcome.
b. Participates in Performance Improvement with active recommendations and follow through.
c. Participates in or takes leadership role in family councils, resident councils, and support groups as appropriate.
d. Assists with scheduling dental and vision doctor visits, reviewing appropriate.
e. Is ready and willing to participate in special projects or committee work as requested by supervisor.
6. Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.
a. Attends all mandatory in-service programs.
b. Adheres to facility policies and procedures.
c. Completes yearly online education program by deadline.
d. Attends employee meetings on a regular basis.
In addition to the essential job specific duties listed above, the Social Worker shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission and values (compassion, excellence, inclusion, integrity, collaboration) of CHI Living Communities and will be evaluated on such basis. Furthermore, every employee must abide by all campus, departmental, and safety policies, rules, and regulations.

Education: Bachelor’s Degree in Social Work from an accredited college or university preferred. Must be a Licensed Social Worker.
Experience: Previous experience with mature adults preferred.
Attendance: Regular attendance is an essential part of this job.
